Eligibility Requirements

 

Eligibility Criteria for Entering Our Fellowship Program:

  • Fellowship applicant eligibility guidelines are outlined in Section III A of the ACGME Program Requirements. We also accept applications from exceptionally qualified individuals as described in the ACGME program requirements
  • Must have completed an ACGME accredited residency program in family medicine (program is currently not taking applicants from internal medicine, emergency medicine, or pediatrics)
  • Must demonstrate a strong commitment to Primary Care Sports Medicine
  • Must demonstrate experience in sports medicine, which may include sports medicine rotations, team or sporting event coverage, or research in the field
  • Show a commitment to scholarly activity through presentations, publications, or resident research projects in sports medicine
  • Applicants must have passed all stages of the USMLE or COMLEX exams within three attempts
  • All applicants must be board-eligible or board-certified on the first day of fellowship
  • Provide letters of recommendation from current program director and other individuals, including but not limited to sports medicine/orthopedic providers
  • Must hold an unrestricted training license

 

Required Submissions

- Apply through the Electronic Residency Application Service (ERAS)
- Current CV
- Personal statement
- Three letters of recommendation from persons familiar with your clinical performance, at least one of these must be from the Residency Director
- Certification of previous Residency Training or Validation of Current Training
- Copy of present state licenses
- Recent photograph

 

Interview Process

Candidates will receive an invitation to schedule interview days through the ERAS self-scheduling tool. During interview days, candidates can expect interviews with faculty and fellows along with tours of our facilities.

Lodging will be provided on a first-come, first-serve basis, and candidates will have the opportunity to dine with members of the fellowship the night before the interview.
