This program offers personalized support to help participants successfully monitor their blood pressure.
High blood pressure puts individuals at risk for stroke and heart disease, and can lead to death. Research shows that recording blood pressure at least twice per month for four months can help to lower blood pressure.

The diabetes prevention program is a research-based program focused on healthy eating and physical activity. Through participation, this program can help decrease the risk of developing type 2 diabetes.

Navigating life after cancer often finds survivors overwhelmed and fatigued. This program helps cancer survivors can get back on their feet through customized exercise plans to improve physical well-being and quality of life. Program instructors are certified fitness instructors - and trained in cancer survivorship, post-rehabilitation exercise, and supportive cancer care.
Participants will:
