Leaving the hospital

Adena takes great care to ensure your ongoing health after you’ve received treatment. If services such as nursing, therapy or medical equipment are needed for your continued recuperation at home, a case manager or social worker may be involved in helping to arrange this for you.

Any prescriptions for medications to be taken at home can be filled at your pharmacy of choice. If you have any questions or concerns, regarding your prescription, you should speak with your physician or nurse prior to discharge.

On your discharge date, please arrange for someone to drive you home prior to 11 a.m.

Once you arrive home, carefully follow any instructions given to you by your physician or caregiver. These are specific to your situation and will help in your full recovery. If you find your condition is not improving, contact your doctor. For any life-threatening emergency or symptoms such as fever, sudden numbness, sudden confusion, blurred vision, sudden trouble walking, severe headaches, or chest pain, dial 9-1-1 immediately.
