Community Health
Needs Assessment

Help shape the future of health in your county

Every three years, Adena Health partners with local organizations to conduct a Community Health Needs Assessment (CHNA) to better understand the challenges and priorities facing our communities. Your feedback plays a key role in shaping programs, services, and partnerships that support better health, right here at home.

By completing a short, anonymous survey, you’re helping us take meaningful action to improve health and well-being in your community.
