Adena Medical Group accepts patient referrals and appointments through several options.

To best care for your patient, the following is needed:

• Patient Information: Full name, date of birth, and gender.
• Referral Details: Specialty, Reason for Referral, Date of referral.
• Referring Care Provider: Name and Phone Number include Practice Name
• Provider for Results: Name of provider responsible for receiving results.

Requesting a patient transfer

2. Provide patient details:
   • Chief complaint
   • Medical history
   • Most recent vitals and labs
   • Pre-transfer treatment course
   • Proposed patient care plan
3. Finalize: collaborate with the Adena team to finalize the patient’s transfer plan
