A CHNA is a tool that helps identify the biggest health challenges facing our communities. It gives hospitals, public health departments, and local organizations the information they need to make meaningful improvements.
The CHNA helps us:
- Understand what our communities truly need.
- Focus on the most urgent health issues.
- Promote fairness and access to care.
- Meet nonprofit hospital requirements.
Most importantly, it gives you a voice in shaping health care where you live.
CHNAs are completed by organizations that care about community health, including:
- Nonprofit hospitals like Adena Health
- Public health departments
- Local coalitions or nonprofits
Every three years. This regular cycle helps ensure that health strategies reflect what’s happening right now in our communities.
A CHNA includes:
- Community health data (like rates of chronic disease or access to care)
- Demographic information
- Local resources and services
- Feedback from community members and partners
- A summary of the top health priorities to address
We listen in a variety of ways:
- Surveys (like the one you're invited to take)
- Focus groups and interviews
- Town halls and community meetings
The results guide real action. We use the findings to:
- Create a plan to address top health concerns.
- Partner with others who share our mission.
- Measure progress and adapt as needed.
Most people complete it in about 20 to 25 minutes.
