  • Promote more informed patient choice
  • Provide feedback to stakeholders and providers
  • Help improve quality and lower the cost of care
  • Improve oversight with respect to Adena Healthcare Collaborative, LLC's compliance with program requirements.
  • This page provides important information for the public about Adena Healthcare Collaborative, LLC.

Providing the full spectrum of health care services within the Adena Health System is consistent with the stated purposes of the Medicare Shared Savings Program and the mission of Adena. In-network care coordination provides more efficient and effective care for our patients. As such, the governing body of Adena Healthcare Collaborative, LLC has determined that, in order to further the interests of the Accountable Care Organization (ACO) as a whole in meeting the objectives of the Medicare Shared Savings Program, it is reasonably necessary for Adena Healthcare Collaborative, LLC to include an in-network care coordination component and care coordination engagement factor in the Adena Medical Group. The in-network care coordination component and care coordination factor will be made available to all eligible Adena Healthcare Collaborative, LLC ACO participants and ACO providers and suppliers.

Please note, the ACO-40 Depression Remission at 12 months quality measure is not included in public reporting due to low sample size. The Centers for Medicare & Medicaid Services (CMS) also waived the requirement for ACOs to field a CAHPS for ACOs survey for PY 2020 through the Physician Fee Schedule Final Rule for Calendar Year 2021.

Additionally, CMS reverted ACO-8 Risk-Standardized, All Condition Readmission and ACO-38 Risk-Standardized Acute Admission Rates for Patients with Multiple Chronic Conditions to pay-for-reporting, given the impact of the coronavirus disease 2019 (COVID-19) public health emergency (PHE) on these measures.

Note: In the Quality Performance Results file(s) above, search for “Adena Healthcare Collaborative, LLC” to view the quality performance results. This ACO can also be found by using the ACO ID A2628 in the public use files on data.cms.gov.
