If you have a question or complaint about any part of your care or hospitalization, please talk with your nurse, doctor or the manager of the department where you are receiving services. If the issue is not resolved to your satisfaction, you have the right to contact the Patient Advocate at (740) 779-7364 or 1-877-779-7364. You also have the right to contact the Ohio Department of Health, Division of Quality Assurance, Complaint Division, 256 North High Street, 3rd Floor, Columbus, OH 4326, 1-800-342-0553, 8:00 a.m. to 4:00 p.m. Monday through Friday.
